UNDERSTANDING THE SAP PROCESS
The SAP process is a structured path — not a punishment. It ensures workplace safety and compliance while giving you a clear, supported route back to safety-sensitive duties.
FIVE MILESTONES
Each milestone moves you closer to your return-to-duty test and back behind the wheel with confidence.
A comprehensive face-to-face evaluation of your substance use history and the circumstances leading to the violation. This includes a clinical interview, a review of testing circumstances, and I will contact the MRO if needed.
Specific recommendations for treatment or education tailored to your needs.
I track your progress through the recommended program with regular provider communication and check-ins, ensuring requirements are being met along the way.
A second evaluation verifies you've successfully completed all recommendations. This includes a review of your completion documents to determine readiness for return-to-duty testing.
I handle the final regulatory documentation so you can get back to work. I will send reports to your employer, manage the Clearinghouse updates, and establish your follow-up testing schedule.
FMCSA CLEARINGHOUSE
The FMCSA Clearinghouse tracks drug and alcohol violations for commercial drivers. Here is exactly how to designate me as your SAP inside the portal.
Go to the FMCSA Drug & Alcohol Clearinghouse and log in using your Login.gov credentials. Once you're logged in, you will arrive at your Driver Dashboard.
LOOK FOR: Driver Dashboard
From your Driver Dashboard, look for the section showing your Clearinghouse status or Return-To-Duty information. If you have a “Prohibited” status because of a drug or alcohol violation, your Clearinghouse record will show information related to the violation and the Return-To-Duty process.
LOOK FOR: Prohibited Status / Return-To-Duty
Select the violation displayed in your Clearinghouse record. Review the information to confirm that you are working on the correct violation. Your violation is what triggers the Return-To-Duty process.
LOOK FOR: Violation Details
After locating your violation and Return-To-Duty information, look for the option to select or request a Substance Abuse Professional (SAP). Select the option to Request SAP.
When the Clearinghouse asks you to search for a SAP, enter:

Dr. Regina Gullette SAP Services provides remote telehealth evaluations. Services are strictly provided for individuals physically located in the states where Dr. Gullette is licensed or registered (Ohio and South Carolina) at the time of the evaluation.
While we provide expert SAP guidance, the ultimate responsibility for compliance with DOT and employer policies rests with the employee. We are not responsible for employment decisions made by your employer or regulatory decisions made by the DOT/FMCSA.
Evaluations are provided at flat-rate fees. Payment must be rendered prior to the commencement of the Initial Assessment. Completion reports will not be released to employers or the Clearinghouse if there are outstanding balances.
LEGAL
When you book an evaluation, that time is reserved exclusively for you. We understand that emergencies happen, but we require a minimum of 24 hours' notice if you need to cancel or reschedule your appointment.
Because the SAP process requires significant preparation and dedicated time blocks, cancellations made with less than 24 hours' notice, or failure to attend a scheduled telehealth appointment (a "No-Show"), may be subject to a cancellation fee up to the full cost of the evaluation.
If a cancellation fee is assessed due to a late cancellation or no-show, this fee must be paid in full before a new evaluation appointment can be scheduled. Please communicate proactively if you anticipate missing your appointment to avoid these fees.
